We've been having the same problem with our male ragamuffin. It started when we moved house, got better after medication but came back last year when I got pregnant Definitely have him vet-checked to rule out a medical problem. If he's fine, it's probably due to stress - has something changed in your house? New person or animal? another cat invading his territory? Cats don't deal well with change. In this case you could try feliway pheromone diffuser which helps calm them, or there are several medications the vet could prescribe. To get rid of the smell we used a product called "piss-off" (yes really!!!)- it works.

i was watching a show the other day and it said that if you get 3 parts water and 1 part vinegar, and put it in the spot the cat peed in, it will not only clean it but prevent the kitty from peeing in the same spot. at least that will help you a little bit :)
