Get your passports and dual citizen cards at the Immigration Office of the countries where you are a citizen.

Generally you can get passport application forms via the post office (sometimes you can download them but it depends on which country you're refering to).

There is no dual citizenship card. You can get passports for each country you're a citizen of and have naturalization/ citizenship certificates though.

There is no such thing as a "dual citizen card".

It would also be helpful if you said which country you wished to aquire a passport from.
