to start? i don't want to overdo it at first or plan so much that i am intimidated by the thought of doing the work. but i have to get back in shape. i am starting to get sick from being so lazy.

Well if your willing to spend a bit of cash on getting a personal trainer at the gym thats your best bet because they will be able to tell you how much of each thing you should do without over doing it.

You don't want to be killing your muscles on your first trip because that might put you off going again and it'll at least stop you doing it regularly which defeats the point.

also with home exercise - careful with the amount you do, as i found out my self first time i seem to be able to do loads - next day i could hardly move.

Set your self a limit and do that same number everyday, then you can slowly raise your limit when your body can cope with the increased stress.

I have exactly the same problem. And I'm a repeat offender so I can only tell you what works for me.

I just get to the gym and do what I feel like for 2-3 weeks. Once I get back into the routine of going and doing, I usually work out a plan that works for me.

Again, it's more about getting there than how intense the visit.
