Consider the position of the eyes separately from the Rx.
What *alignment* will they have?
Then apply the correct Rx for that distance centred on that PD
A Plus distance Rx doesn't get a near PD just because it's +ve.

Use the distance pd for both near and distance glasses.
It gives a little help with convergence.
