If you have a regular physician and are old enough to make an appointment and to go then do it. When you are in the room, tell him what you said here and ask for his help. He will turn you to the people that can help.

Get help immediately. Talk to your parents about getting a therapist to analyze what your problems are. That way, if needed, you can be put on anti-depressants. But, sometimes just talking about your feelings can make a huge difference. Start a journal or something to record your feelings on. I hope everything works out for you.

What you need to do is get to a doctor and explain your symptoms really although at the time it may upset you you will get help and a proper diagnoses at the same time then perhaps your doctor may very well refer you to some therapy.
