if you translate word by word is not gonna sound nice but try this:
que simpre sigas bella
or
eres bella
or
bella por siempre
or
bonita por siempre
or
sigue asi de bonita

As you can see, there are many ways to say this, but for a cake, maybe it would be better if you just say:
Siempre bella (always beautiful)
Because in Spanish the literal translation would be:
Permanece bella / hermosa / bonita
Quédate bella / hermosa / bonita
Stay: permanecer, quedarse
Beautiful: bello, hermoso, bonito

quedarse is to stay so you want to write it as a command. For someone you know really well you just conjugate it as the third person. I am guessing this going to be for a girl or women so you should say, "se queda bonita"
