Also, some schools have information on insurance plans for athletes that are cheap ($30 a semester)

Urgent care clinics will often do these types of exams for around $100. Sometimes there is a charge for the exam even if you have insurance since the doctors have to fill out different forms.

Those are usually a special price so ask in the school office or the gym class. I think that in a lot of cases they can get a doctor to give you one for free so you can play in sports at school.
