On the container should be the patients name and date of birth. On the lab slip should be the patients name, date of birth, referring physician, test to be done, diagnosis, and the physicians signature.

specimen labelling per our medical laboratory protocol:
patient name
unique identifying number (i.e. date of birth, medical record #, etc.)
requisition information per our medical laboratory protocol:
name of physician ordering test
date of collection
type of specimen (clean catch, catheter, bladder tap)
patient name
unique identifying number
date of birth
sex
(also ideally: list of antibiotics patient has recently taken, as this may affect culture outcome, and pt. diagnosis)
