It's not a name for an American, but a white person. You can say guero as well as gringo. A white American would be called any one of those things if say they were in Mexico or by a Mexican person.

A term that is less pejorative than "gringo" is "anglo", for an English-speaking US citizen.

In England they say "Septic Tank" or just "Septic" because it rhymes with "Yank". In Australia they also say "Seppo".
