Ask if they have a company change of address form...if not, email them saying you have moved and here is your new address. Print out the email and save it for your reference. If they failed to send you your check in a timely manner, you might be able to receive some type of compensation for their failure to send you your check.

Send them a letter certified mail, return receipt, advising them of your new address. Stop doing it over the phone. If they still refuse to send the check tell them you will report them to the state department of labor, then do it.
