My guess is: one.
As a medical school student on an internal medical rotation, many years ago. We had little supervision. I remember receiving an 80 year Cuban gentleman as a patient. He was unceremoniously discharged from another hospital to go home to die. He was comatose in liver failure from alcoholic cirrhosis. We, as medical students, (our supervising resident was worthless) got his ammonia levels down, and he woke up. He later died from complications of our manipulating his fluids in a condition called "hepatorenal syndrome". I feel responsible for not preventing it. I did, however, keep him alive long enough to see his grandchild born and named after him. I always keep him in mind every time I meet an illness with which I am unfamiliar. Since that time, I have saved hundreds of lives, and have not killed anyone else, to my knowledge.

It depends on what you mean by kill.
I have never deliberately killed a patient.
There have been perhaps 3 to 4 patients who perhaps might have survived if I had managed them differently, something I realized later.
There may have been some patients I could not save who could perhaps have been saved if they had gone to a hospital with better facilities. There may be other surgeons who might have been able to save some of them.
I suspect there are no surgeons who cannot say the same.
I don't worry about these things. I just do what I think is the best for the patient.

It also depends on the field of medicine.
Someone who is a trauma surgeon is going to lose a lot more people than someone who is internal medicine.
It is NOT fair to call this "killing". You make it sounds as if it is a premediated act and that the doctor didn't try/doesn't care/wanted to kill.
