While Judy is correct, here are examples of what I would expect to see. Goals need to be specific, attainable and measurable. You can't just say, "I plan to lose weight." Be specific, I plan to lose 5 lbs. in 1 month.

Describe the goals you had set out to accomplish for the year:

- To become Microsoft Office Certified by June of 2007.
- To find an Association to get involved with for networking and further professional development before year end.
- To have all contract files scanned in by March of 2007.
- To hold a training session for all employees on how to deal with difficult people by August of 2007.

Which goals did you accomplish?
- I became Microsoft Certified on May 28, 2007.
- A training session for all employees on how to deal with difficult people was held on June 3, 2007. We had an outstanding attendance of 60 employees.

Which goals did you not accomplish and why not?
- I have not found an association that interests me. I found several that were promising, but did not feel they met all of expectations.
- The contract files have all been scanned and filed, but was not completed until May 17, 2007 due to a higher priority project that came up.


Which achievements are you most proud of?
- I received my Masters degree in February.
- I became Microsoft Certified.


What are your expectations for the next evaluation time span?
- To create new goals that will better my professional development and have a lasting effect on the company's success.
- To discuss any improvement areas

What can your manager do to help you achieve your future goals?
- Support me in my educational and professional development initiatives
- Allow me to become more involved in top projects such as mergers and acqusitions or risk analyses.

What are your goals for the next evaluation?
- Take some courses on Risk Reduction by February 2008.
- Attend a business seminar by May 2008.
- Improve our risk anlaysis process by end of year.
- Increase our operational efficiency when acquiring new companies by end of year.

What I would say would have no bearing on what you should say, since it depends on the individual job and you have to answer for YOUR job and accomplishments. It's a very reasonable list of questions.

Whom you want to evaluate ? yourself or me ? If I answer these questions, it will be my evaluation not yours.
