If it is in Europe then the table wear is from the outside to the inside for each course, Europeans don't generally eat salads as a first course. In France the table wear is placed upside down, i.e the prong of the fork in downwards. The glasses are from the right, water, white wine and then red. In the USA it is regarded in the best of houses that the paper plates are stacked in the middle along with the 6 gallon tub of KFC and fries, ( assuming there are 3 to 4 people attending), ensure that the oilcloth on the table is reasonably clean and a plentiful supply of bibs and kitchen roll is available. If anyone farts or chucks up, accept it as part of the culture. Oh yes, they seem to have salad with everything including hot food, how bizarre. Har de har har har, And before anyone starts to jump up and down with indignation remember to have a sense of humour and try to laugh at your self.

Depends to a degree how formal and if it is American or non American. Assuming American and not extremely formal......

salad fork - dinner fork plate knife, spoon for coffee, spoon for soup.

The wine glass is at the tip of the fork and the water glass to its immediate right.

Think about the order in which you eat the food. The utensils you would use first are the furthest away from the plate.

In the US dessert utensils are brought afterwards. Many countries put them just over the top of the plate.
