What are the best 5 casinos in Puerto Plata with a lot of slot machines??
I prefer 4 star hotel casinos and above...

None that I know of that are 4 star in Puerto Plata.
There are two in Punta Cana
Grand Palladium palace Resort Spa & Casino
Barcelo Bavaro Palace
3 Star in Puerto Plata
Allegro Playa Dorada
Breezes Puerto Plata Resort Spa & Casino
Paradise Beach Resort & Casino
That is all I know of.

Wow interesting question. I don't think you are going to find any 4 star or above with a casino in Puerto Plata let alone 5.
Best you will find are legitimate 2-3 star which will be in Playa Dorada. If they claim more stars they are fibbing!
