You don't have to be English to play for England, you just have to have an English grandpaerent or have a british citizenship, e.g John Barnes was British not English

Because the manager doesn't play. For countries that are very small e.g. Andorra, Faroe Islands they probably haven't got any decent manager so need foreign coaches. You can't have different rules for different countries.
