I had a conflict at my ex-job and they put out a credit card abuse warrant for me I am working to get enough money to fight the case but until then they do not know my address & I do not want to contact them how would I get my W4 form?

You can either put in a forwarding address with the post office so that if it sent to your old address then it will get to the new one. or......Get a Post Office box and submit a change of address form to the company by certified return receipt mail.

W4- is the withholding document I don't think that is what you are looking for.
You are probably wanting to know how to get your W2( that's what you file taxes with)
W2's can not be run prior to the last pay day in Dec
W2's are run at the end of the payroll year and by law must be released to all employees no later than Jan 31.

You will have to give them your correct address if you would like your W2 mailed to you unless you put in a change of address when you moved from your previous address in that case the post office would just forward it.

http://www.irs.gov should have a downloadble version of the current W-4, which is what your employer uses to take taxes out of your check.

A W-2 is what is sent out in January, by law. They will send it to your address on file, and it should be forwarded to you, if you have left such instructions with the USPS.

Otherwise, you are stuck.

All the remedies begin with contacting your former employer.

Notify them of an address change and have mailed to a parent. THey may still hold it based on your non-payment.
