You pay with a credit card. Load a Web page over their Wi-Fi connection, and you'll see payment instructions.

The McDonalds I have been to and used their WiFi they had a WEP key and SSID they gave me to get on and I was never charged, probably depends on the store. I had to ask the Manager to get this information as no one else knew it.
