The sentence is fine, but
"How long have you had pain in your left shoulder?" could be better.

To get technical: For how long have you had pain in your left shoulder? You don't need to say 'this'. since pain is abstract concept. Using 'this' implies a concrete noun; something that can be seen or touched.

It's colloquially correct. To be absolutely correct, you would have to say: "FOR how long....?
