Never install a wall mounted switch ina bath room - so not C.

Ill assume that since D specifies a pull cord and question A and B dont then A and B will be wall mounted as well, so install D - a double pole pull cord switch.

None of them A ceiling mounted double pole switch may be mounted within 2 meters .A wall mounted double pole switch can be used to isolate the shower but it must be outside the room or at least 3m away from any location likely to have water present ie shower bath etc

The switch does not have to be in the reach of the person in the shower.
