Everyone with A military Email has it assigned to them, Mine uses my middle initial.I have a Friend who uses only his first and last name.

Depends on the command's procedure.
At every command I was at, it was ranklastname@command.service.mil if it was a common last name it was ranklastnamefirstinitalsecondinitial@command.service.mil.
All military E-mail addresses will end with a .mil.

It all depends on how common their name is. It could be just the first and last, or first...last...middle initial.
