Green witch and hedge witch are pretty much interchangeable terms. They are usually solitary practioners.
Their self-description would be:
*Green Witches are allied with the green growth of the Earth. *A Green Witch's heart is as green as the heart of the Earth. *A Green Witch weaves the ways of the green nations with the ways of humans.
*A Green Witch understands everyday magic.
*A Green Witch creates green magic: heals with common plants, simple ritual and compassionate care.
The ways of the Green Witch are individual and water like; each personal stream joining the one ocean of love.

I second Kallan's very well spoken answer, and would add that I bet Raji could give you some really good insight about it. My impression of being a Green Witch is a particular affinity to herbal lore and plant life, which many Witches do have, but perhaps you could call this a main focus of being a Green witch.
I have a book called Green Witchcraft which discusses these things in detail. I recommend it if you want to get a lot more information about it.

As others have pointed out, you seem to be confusing Wicca and witchcraft. Calling oneself a hereditary Wiccan would demonstrate a lack of understanding about what Wicca is. However, there are hereditary and fam trad witches and they every right to call themselves that just as I have every right to say I am a descendant of Ulysses S. Grant - because I am. Hereditary Witchcraft does exist, though not to the extent that some claim - but they do exist. We've at least one on this board I can think of off the top of my head. You are correct that Wicca is a modern religion and there are no surviving families of Wiccans dating back 100 years. Some of the sillyness surrounding the fam trad stuff is the fault of early Wiccans and then there is also people like Gwen Thompsom who claimed a family tradition yet published The Rede of the Wiccae* which gives the last line as "Eight words the Wiccan Rede fulfill: As Ye Harm None, Do What Ye Will" It is these types of things that blurred the line, and yes confusion abounds. Yes, a good number of those calling themselves "hereditary" are full of bs. But some are not. Take the time to listen to what people are saying - if you are a smart one, you will be able to determine who is legit and who is not.

A Green Witch usually works alone, interacting primarily with the natural world. Historically, a green witch lived apart from the rest of society and used the energies of plants and trees around them to heal others. The Green Witch was sought out rather than sought those who needed healing. The Green Witch is well versed in healing properties of plants, herbs, trees, fungi, and other living things and used those poroperies not only as healing potions and remedies but ALSO as a way to connect him.her self to the life force of the Earth. The path of a Green Witch consists of BOTH witchcraft and shamanism in that not only are herbs used directly on the illness but they are also used to "charge" the spirit of the ill perspon with healing spiritual energies as well. Therefore a Green Witch is one who lives on the Green Path and is aware of how energy flows through life and the environment and is able to use that energy to accomplish healing on the physical level, the mental/emotional level AND the spiritual level, ALL at the same time.
