Some doctors can now exam the retina in a non-mydriatic way (i.e., without dilating your pupils). For example, at the practice where I work at, we use the Optomap, which is a digital scanning ophthalmoscope. Nine out of ten people don't have to be dilated and it's quite interactive for the patient since you get to see your retinas.

Having said that, I'm not sure what the laws are where you live but I believe here in Ontario, when a person books an apppointment to have a full eye exam the doctor has to check the retina one way or another (be it via dilation or digital photography) b/c obviously s/he is liable if they miss something that may lead to serious problems.

You can always decline a procedure at a doctor's office. Just make it very clear that you don't want to have the procedure done. Just understand that you will not be receiving as thorough a comprehensive eye exam. Without some other instrument to check the posterior surface of the eye (OCT, Bscan) there will be no way for your optometrist to effective tell if there have been any changes over the past year. And noting those changes early can help prevent degenerative problems in the future. If anyone in your family has a history of retinal problems, macular degeneration, etc. I strongly recommend dilation. Just plan properly for your next exam and don't plan on doing any near work for 4-6 hours after the exam.

Most Doctors require dialation currently, because while it does not have anything to do with the rx you get, it does allow the Doctor to get a better look at the internal health of your eyes. I know, I hate it as much as you or the next guy, but this is a good thing too.

Dilating is necessary for the doctor to get an accurate idea of what is going on with your eyes. Your vision will be blurred for at least a few hours, but it won't cause any permanent vision problems. Some offices even carry drops that can reverse the dilation.

That wouldn't be a good idea. Not allowing them to dilate would be like not opening your mouth enough for the dentist to see all of your teeth.

Next time you're scheduled for an eye exam, call ahead of time and ask if they'll be dilating. Not all eye exams require it. If it is necessary to dilate, arrange for someone to drive you there and back, and try to stay out of bright light as much as possible.

Yes, having your eyes dilated is annoying. But it only lasts for a few hours. And it's a small price to pay to maintain your eyesight.
