i had worked for the same firm for several years and one morning when i came into work my boss already had company in his office.
i had never met her before, i assumed it was a potential customer and once the boss and i were alone i made mention of her overwhelming perfume by stating
'god it smells like a chinese whorehouse in here'
well it turns out the lady in question was his wife who he was separated from and they were trying to work things out.
he was a little surprised by my comment but i didn't suffer any repercussions other than extreme embarrassment over my putting my foot in my mouth!

Deleted an email I sent to my boss.
I went to her office and logged on to her machine and I didn't know when she would be back, in a busy office full of people who knew I didn't belong on her machine.
(Man, my answer sucks compared to these. Some of these are AWESOME!)

Sprayed a fire extinguisher and then jumped out of a second floor window all caught on security tapes. (Was Never Fired)

stood there in front of my boss with a blank mind when he needed me to do something so simple and he ended up huffing and puffing and doing it himself. i was so embarrased
