My guess is the Vatican Guard. They are there mostly symbolically to protect the Pope and the Vatican. I dont' know much about them but I don't think they have changed in a long time (again because they are not really a modern army but a symbolic one)

I'll say Paraguay cause of the list. Granted this list only ranks according to size but if a country has a small army then I think that the founding is almost nil. Without proper founding then they become outdated in a couple years.

Tibet, it has a few guys in old style uniforms and non-working guns, and the entire country is occupied by the communist chinese, just to keep the peace you understand, nudge nudge wink wink.
With regards the Vatican Guard answer it is not an Army, even though most of its members are drawn from the professional Swiss Army

American Army:
To much dependence on technology.
It lacks the ability to adapt and carry out meaningful asymmetric warfare.
Morale is susceptible to slight whims and politics.
