YOU HOLD THE PICK WITH YOUR 3 FINGERS YOUR THUMB
YOUR FIRST AND SECOND FINGER IT ALWAYS DEPENTS ON WHICH KIND OF PICK YOU USE TO MAKE A LOUDER NOTE ON YOUR GUITAR

I was never successful at using a straight pick when playing. I initially resorted to thumb and fingers only (fingerstyle) and then graduated to the thumbpick when volume was necessary.

The few times I tried a straight pick the durned thing would fly off to netherland! Yet many pickers will glue a small spot of coarse sandpaper on the pick's side for a better grasp of thumb and fingers.

I took classical guitar lessons for 5 years, never able to use a pick. The great thing is, you don't havta if you don't wanna, I found it faster and easier to use the old p-i-m-a method, much more efficient that trying to use a little plastic tooth to match the coordinated movement of fingers

It's usually held between the index and the thumb, how guitarist go fast is they use that up down method....up down, up down, up down, over and over again....this is the way they achieve speed....I'm sure that there are people who use a different method....but this is the standard....I can't use a pick....I'm classically trained....and use my fingers...it's very hard to make the switch. I admire people who can use the pick ....especially the ones who have the kind speed your talking about...like Eddie, Slash, and SRV.
