my former employer who went out of business placed incorrect information on my W-4 which was sent to the IRS. Instead of 2,900 she placed I earned 29,000. How do I fix this? My former employer acknowledges doing this but failed to fix it. What do I need to do.

I assume you mean the W-2 she created, not the W-4 (you filled this out when you started working there.) Your energy is best spent getting her to generate a corrected W-2. And it's in her interest too. If she reported that she paid you $29,000 in wages but only sent in payroll taxes on $2900, the IRS is going to be looking for a lot of money from her. And it's money she will be personally liable for, even if the business has shut down. The IRS also has penalties for not correcting an incorrect W-2.
If you cannot get her to do anything, only report the income you actually received and attach a statement to your return explaining the discrepancy. And in the mean time call the IRS and explain that your former employer won't correct your W-2.

Do you have a paystub showing the correct YTD amount? Talk to the IRS and explain your problem to them.
