With I-485, you need to include I-134 Affidavit of Support. and I-693 Medical Examination of Aliens seeking Adjustment of Status. You need to see a doctor designated by USCIS and the doctor will fill out the I-693 from. You can get the list of doctors around you at the USCIS website.
You can also file I-131 Application for Travel Document, since the filing fee $1010 includes Biometrics, I-485, I-765 and I-131. With the I-131 Approval your spouse can enter US sooner and wait in US for the adjustment.
All the immigration forms come with instructions which you can download in set from the USCIS website. www.uscis.gov and click on Immigration Forms.
If everything seems still confusing, consult with an immigration attorney. Initial consultation costs from $150-250 per an hour depends on your location and you can get tons of informations. You don't have to make a decision to hire them at the initial consultations. Good luck.

You will get more accurate answers in a more timely manner if you just ask the USCIS at 1-800-375-5283. The TTY number is 1-800-767-1833.

Dont forget the Affidavit of Support to show that you can support your wife financially as well as corresponding W-2s and tax forms.
