The penalty points are valid for three years but need to remain on your licence for four years. After four years, if you wish you may apply for a new counterpart licence that will not have any record of the points on it.
You will need form D1 (previously D750) and a fee of £19.

Generally when you reach 12 points you will receive a driving ban. I have known people to exceed this and not be banned. They have gone to court and pleaded that they have some exceptional circumstance and as a result the magistrates were lenient and gave further points instead of a ban.
When your ban has expired you must apply for your licence back. It does not come back automatically. When you do receive your licence the points will have been wiped off.
When obtaining insurance make sure that you declare the disqualification because if not you will commit an offence of obtaining insurance by making a false declaration (something which you could be arrested for!).
In relation to receiving points on your licence and not totting up, the points are valid for 3 years, however you cannot have them removed until after 4 years.

The question you ask is a good on! lol In all seriousness though it is such a broad question. Totting up is when you get 12 points on your licence and a court disqualifies you. However...you can have over 12 points, it is up to the discretion of the court.
Speeding points/running a red light/using a mobile while driving...these type of offences carry 3 - 6 points and are VALID on the licence for 3 years BUT will show on the licence for a further 12 month period. Drink driving and dangerous driving conviction will be VALID for 10 years on the licence but will show for 11 years.

I understand that once yo are banned you start again so far as points are concerned. However I think the Insurance would still want to know about it because you would be considered a bad risk

This depends....how many different drivers licenses do you have and from what states? Illegal immigrants have an average 10 to 12 drivers licenses from different states! You can buy them on the Internet......there are illegal immigrants working at drivers license bureaus...under false names and make a bunch that they take home and sell all the time! The US government knows it , but to say anything to them is racial profiling and that is not going to happen!
