This type of aggrevation only happens when I sit at my desk at work all day - I get up and move about several times a day but the soreness and pain and headache are almost on a continual basis - how can I avoid this?

Well maybe after couple of hours stand up and have little stretch, exercise your neck a bit.

Adjust the chair comfortable so you can lean back on it and relax bit more could help. I sit on my desk comp chair and work often too so thats how I make myself comfortable from those sore aches.

You have to stretch and move while actually at your computer. Also, if the middle of the monitor is not at eye level, that could cause problems too - it might need to be adjusted for that by being raised or lowered.
