Jeremy, I have struggled with one great sin - I have done it only 3 times but it was HUGE. The last time the grief in me was so overwhelming that I felt the same way as you. I did not realize how "sinful" I was being until this happened. Maybe you needed to go through this to get a really good grasp on how great the sin is that you were doing. Now, that you deeply want the forgiveness you want to have this "yoke" lifted. Though, you have suffered through this I think some of it may be satan trying to deceive you. Appreciate, where you are at with this deep grief (I know how can you right?). Well, you are feeling this so deeply that perhaps this is God helping you grow your relationship with Him. By this deep sorrow you will no longer want to commit this sin (I hope). I can tell you I DO NOT want to do the sin that seperated me from God EVER again. Not that I am saying satan won't try to tempt me...but the grief I experienced was so deep - I don't want to ever go there again. Trust God. He is still leading you through this. There are parts in the bible where He turns away from sinful Israel and takes himself away for awhile and they feel it and are saddened but remember He is faithful and does always return. You are growing in your faith stay strong on your path. Believe you are still going in the direction you should be going (though it feels like night / darkness - refer to some psalms about this). If you were a Catholic I would sugest you receive the Sacrament of Reconciliation.
Reconciliation is a sacrament meant to be a healing encounter between God and the penitent, and there is no need to worry about exactly what to say or do.
This is one way how to go to Confession:
+ Get yourself ready by making your examination of conscience.
You enter the confessional. You can choose to talk to the priest face to face or anonymously behind a screen.
+ After the priest greets you with the Sign of the Cross make the Sign of the Cross and say: “Bless me Father for I have sinned. It has been (say how long) since my last confession. These are my sins…”
+ Tell your sins simply and honestly to the priest. Don’t be embarrassed.
+ When you are done listing your sins please say: “…I am sorry for these sins and all the sins I can’t remember.”
+ Listen to the advice the priest gives you and accept the penance from him. Then make an Act of Contrition for your sins:
My God,
I am sorry for my sins with all my heart.
In choosing to do wrong
And failing to do good,
I have sinned against you
whom I should love above all things.
I firmly intend, with your help,
to do penance,
to sin no more,
and to avoid whatever leads me to sin.
Amen.
+ The priest will then conclude with the words: “Your sins are truly forgiven, Go in Peace”. You respond: “Thanks be to God.”
+ Perform your penance as soon as possible.

If you are deply grieved as you say you are they it shouldn't be too difficult to pray...in all sincerity to God.
Sit or kneel...where ever you want to do it as long as you deeply and sincerely want to confess your sins to God go ahead and do it. Relax...take a deep breath, calm your mind and body...think only of God and His love for you. Then when it is right just pray. Don't mumble out a repetitious prayer (that you are used to in church) instead say what you must from your heart.
Let your heart speak to God. Let your heart pour out its pain to God. Be honest holding nothing back. When you have done this you will feel God's warm hand embrace your heart and you will know He is with you.
I do exactly the same thing. Find a comfortable place for you to speak to Him. But speak your heart. God bless you.

Friend, You are trying to pray & yet not getting no where, Let me say, when you pray & ask God to forgive you, Believe him with your heart that he has forgiven you, You are letting satan lie to you, Salvation is not only by feelings but by Faith. Did you know that the plan of salvation was Finished on the day of Pentacost, First you need to repent, Jesus gave up the Ghost & just before he died, Jesus said IT is finished, what is finished, our salvation & the price that was to be paid for our salvation, so Friend First repent & trust God, Then be baptized in water by complete immursion in the name of the Lord Jesus Christ & you will if you are honest with God receive the baptism of the Holy Ghost(that completes the New Birth,) Your salvation is not something you have to beg for, Just come to God just as you are, & then let God lead you everyday & you will grow in God. Remember salvation is not always feelings, when you have no feelings, It is then by faith we take God's word.

Have you read the bible ? Every day ? Romans 10:17 " So then faith cometh by hearing, hearing by the word of God ". Go to a bible believing church to be fed Gods word and to start to serve him . Tithing , supporting missions, witnessing to others , having fellowship with other believers Hebrews 10:25.That will help you grow in his Grace

It is so easy Jeremy!! All you have to do is believe in the Lord with all your heart and trust Him! Believe that He died for your sins and rose again. Ask Him to forgive you for your sins and come into your heart! He will wipe away all your sins as if they never happened and will remember them no more!! God bless you!!

My dear friend, what you are experiencing is referred to as 'godly sorrow' - it's a special kind of grief that the Holy Spirit of the Living God places in us, in order for us to come to a level of repentance.
The Word teaches us that, "Godly sorrow brings repentance that leads to salvation and leaves no regret, but worldly sorrow brings death."
2 Corinthians 7:10
God wants us to know how much sin HURTS Him. It grieves God so much when we continue to rebel against Him and disobey Him. God gave us His BEST - His one and only Son JESUS - so that we may be reconciled back to Him. The only way we can truly be ACCEPTABLE in God's sight is through FAITH in JESUS.
JESUS loves you, Jeremy. He has NEVER given up on you. It's not God's will for you to perish in sin.Thats why He is allowing you to experience 'godly sorrow', so that you can remember how ugly and terrible sin is - and so that you can return to Him once again. Read the story of the prodigal son - God will welcome you with open arms when you return to Him once again. He will wrap you in His arms and cleanse you completely and clothe you with His robe of righteousness.
All you have to do is call upon the name of JESUS CHRIST. You have to have FAITH. You cant make yourself clean. You cant earn your way into Heaven just by doing good stuff. On our own we cant make it. Thats why we need JESUS. When we accept the sacrifice He gave on our behalf - when we receive Him into our hearts and our life - we make a covenant with Him - allowing Him to take LORDSHIP. There is REAL POWER in CHRIST.
Just cry out to God. A broken and contrite heart God will not despise or reject. He wont reject you, Jeremy.
Just talk to Him as if He was seated right across the room. Talk to Him as if you could reach out and touch His face. Tell Him, honestly how you feel. Tell Him about your struggles. Tell Him how much you need Him. and how much you want to know Him. Tell Him to show you His POWER. He will, Jeremy. Just have FAITH and He will help you.

Interesting how you said in your question that you've already prayed, yet everyone tells you to pray more! lol
How to repent:
Stop doing it.
To repent means to turn in another direction. So you literally have to turn your life around and go in another direction. Like say you are someone who likes porn. To repent would be to stop looking at it, turn your mind to God rather than what you see online.

Prayer is what you do, say sorry for your sins thank him for sending jesus, say you believe in him and dedicate your life to him, invite the holy spirit into your life to help you live your life for God!
Get involved in a church to help you and to share with others.
