Why is she taking the painkiller? You did not say why. Have they been prescribed by a doctor for some current or chronic condition in her? Where's the pain? How long has she had it?
All these are things which only she (and you might know), should be answered by her herself and her doctor. Being in the condition she is with those other illnesses, she will do much better going for an earlier visit to her doctor. If the pain does not go away within those 30 minutes as you've mentioned, she should be wondering why, and going through emergency room at hospital if it persists.
PLEASE be careful with self-prescribed medications.

If you are concerned about it, you could phone a pharmacy, and they should be able to advise you. Or you could phone an out of hours emergency service. If you live in the UK, try phoning your GP. They will probably have a recorded message which will tell you where to get advice. If that doesn't work and you are still worried, if you phone 999, they should be able to help.

painkillers can take an hour if not more to start working, as long as you dont exceed recommended daily amount there is nothing to worry about. paracetamol and ibuprofen can be used together to try and get the pain under control

Be VERY careful what you mix and take, just be sure to follow doctor's orders, or make sure what you are mixing is ok.
I had a friend who mixed the wrong pills (morphine, xanax, hydrocodone, etc...) and he feel asleep and almost died. He aspirated in his sleep and had someone not been there to take him to the hospital he would have died.
Not to scare you, just be careful, check with your doctor first.

depends on just what they are really - you risk causing bleeding of the stomach, kidney/liver damage and a number of other things with some painkillers. Be careful.
