If there actually were a third world war which was fought as an all out war, the options that mankind had to survive would be extremely limited. Assuming that in the all out warfare waged by the nations involved nuclear weapons. There really isnt anything that can be done at all. The worlds nuclear power combined has enough firepower to blow up the entire planet and then some. The only way it could be fought is if all nations of the world dismantle their nuclear weapons. which wont happen...

Also, people are talking about how humanity would become extinct after a nuclear war. I don't think we would, if you have a look for example at the radioactive zone in chernobyl, animals continue to reproduce and they have animals in the radioactive zone that are close to extinct in other parts of Ukraine, Humanity would lose a lot of population, and a lot of progress, but i believe we are too hardy to be wiped unless the earth itself is obliterated before we can get off it. Apart from that, just remember, we survived a 50,000 year ice age and emerged victorious with many woolly mammoth tusks (but not very many woolly mammoths!).
We would very likely develop small tribal governments- either based on merit, family or age, as these are the best and most intuitive governments for small communities. We would go back to an agrarian existence, albeit with some better engineering knowhow. There would definately be lots of birth defects, and lots of deformities but we'd survive and the fittest of us would procreate, over time we'd adjust better to our environment, knowing which milk (such as goat versus cow, which poor ukranians learnt the hard way) produces the least problems and which fruits and vegetables were the least affected by chemical/radiation. You specified that drinking water would be tainted, that's fine, but there is moisture in everything and we'd find our sources.
With man gone more or less, nature would take over,and re-establish ecosystems within the new environmental parameters. i think that it would be a long long time before primitive cities would eventually take form.
But i do think that that would happen.

Pray. That's about it. If there is a 3rd world war, we won't have to do anything in order to survive.. lol the world will be starting over without us. =)
