You need an ice cream maker. Basically this is a bucket, and inside the bucket is a sealed "can" that has a "beater" inside the can. On top is either an electric motor or a crank mechanism to spin the can and beater (at different rates).
You put the ice cream recipe inside the can. Put the can in the bucket. Fill in the bucket with ice and rock salt. Spin the can/beater (plug in the motor or crank it - when cranking you will want to take turns cranking it). As you go, you will need to periodically add more ice and salt. Salt water has a lower freezing point than plain water, so the salt and water will turn in to very cold salt water (a.k.a. brine). Brine will usually spill over a little bit on the side, so do this in a sink or on a driveway or a similar place where salt water will not harm anything.
Eventually, it will get harder and harder to spin. At some point, the motor will stop spinning. This is because the ice cream mix has frozen.
As far as the recipe goes, you can find several. You probably can find some on-line. Often times, the kind of cook books that churches sell as fund raisers have recipes. Generally they consist of milk, cream, vanilla, etc. Maybe you add strawberries if you want strawberry ice cream, etc. One of the better ones I've had used vanilla pudding mix.

We used this method in my 8th grade Science Class as a lab. It''s incredibly easy and tastes great!
the ingredients are:
1 tablespoon sugar
1/2 cup milk or half & half
1/4 teaspoon vanilla
6 tablespoons rock salt
1 pint-size Ziploc plastic bag
1 gallon-size Ziploc plastic bag
Ice cubes
To make:
Fill the large bag half full of ice, and add the rock salt. Seal the bag.
Put milk, vanilla, and sugar into the small bag, and seal it.
Place the small bag inside the large one and seal again carefully.
Shake until mixture is ice cream, about 5 minutes.
Wipe off top of small bag, then open carefully and enjoy!
Be careful with the ice, sometimes it creates holes in the plastic bag, it can get wet...

If you buy an electric ice cream maker there will be instructions in it but basically you put your mixture in freezer then make layers of salt and ice around outside. Most recipes call for cream but I make a basic vanilla custard-4 cups milk, 6 egg yolks, 1/2 cup sugar, pinch of salt, 1 teaspoon vanilla. Heat milk (not quite reach a boil). Beat egg yolks with sugar and salt. Add milk to egg mixture a little at a time, strain into top of double boiler. Cook over simmering water until mixture coats spoon. Cool, add vanilla, put in ice cream maker and freeze until stiff. This makes a very good ice cream but not too rich. You can add peaches, blueberries, strawberries to cooked custard if you want.
