You do not have to place your name, address, and date in the upper-right-hand corner of a letter. It can be centered or to the left margin in blocked format.
Skip down two lines before placing the date and then four lines to the addressee's information.
End it with Sincerely, Warm regards, or whatever you feel appropriate.

Remember: a thank you letter is intended to accomplish three things: (1) express gratitude for your interviewer's time and attention, (2) reinforce anything from the interview that you feel the interviewer responded well to and/or (3) communicate anything else that you think is important that you weren't able to get to in the interview. Example:

right hand corner goes your address and info on yourself ie phone num etc, n left hand goes the date and any reference numbers, hope this helps.

Sir can be replaced by Madam, Messieurs, Mesdames.
Sir can be replaced by Madam, My Lord, My Lady....... and all that caper
