If you're talking about Blackwater,then the answer is NO ONE!

Technically, everyone involved can be charged. The individual that murders the civilian should know or reasonably believe that the order is wrong (even in other countries, the US frowns on civil rights and constitutional violations, as seen in some cases recently). The CEO can be charged also because he allowed or ordered the civilian to be murdered.

In the first place the person who commits
the murder. Then the person who ordered
it. However. As someone already mentioned.
If your question targets at Blackwater it's a
bit more tricky as Blackwater does not
fall under Iraqi legislation and over here they
have pretty much a card blanche. However.
This is only true for contract work. And I'm
pretty sure driving around and shooting at
random cars for no particular reason is not
covered by any contract.

Well, you will have to offer a link to the full story, all cases are different and merit different replies, dependent on if the alleged crime was committed in the USA, Iraq, Afghanistan, etc...etc...

If you suspect a crime has been committed then you must report it to the relevant authorities to be investigated.

Depends on the agreement they have. Seems like our Military members are charged with crimes all the time even though they are acting on behalf of the military supposedly. For example, Lt. Tally in the Vietnam war. Something about Byzantine rules.
