If they ask, I would pray for them. Many people do ask me, and I always pray for them.

Not offended- when this happens I usually tell them that I am thinking about them and hope all works out okay. I have never had anyone ask this that knows I am an Atheist.

If they got insistent about it, I'd probably say "yes" just to be polite, but I wouldn't actually do it because I don't believe that any god(s) intervene in human affairs.
If they weren't too insistent, I'd subtly shift the conversation by saying something like, "You'll be in my thoughts." Thoughts, not prayers.
