That really depends on the religous denomination-but
Reverend is a title for those who do the work of God. Pastor is usually the leader of a non-Catholic or Anglican church. Some larger churches may have several pastors, with one being senior. Minister is usually a worker or leader within specific ministry of a church like minister of music, or minister to children and so on. In large churches these roles may be filled by a pastor.
The church I am in now has 1 pastor. He performs many duties. One church I was in had 7 pastors and a multitude of ministers, and directors.

First of all...."The Reverend" is a TITLE for a minister or pastor or priest. (Like "The Honorable" is a title for a judge)

It is a mistake to call somebody "a Reverend" - just like it would be wrong to call a judge "an Honorable". It's just not proper.

Pretty much the same...just different names for them. Reverend is more of a title like Doctor would be to a phsycian whereas minister or pastor are used to describe what they do or their function in the church.
