If you go to the supermarket directly just ask any staff over the counter, they should be able to give you an application form, or at least let you know who you can speak to. Generally you would like to speak to the HR or store manager.

You can also speak to your recruitment agent. They are generally quite good in helping people finding jobs.

On-line applications may work as well. Sometimes on-line applications go to the recruitment agent the supermarket has appointed.

Hope this helps.

Go to the supermarket, and ask at the office for an application. If they take applications over the Internet, just fill it out there if it's just one store or a regional chain - if it's a national chain, I'd go to the store and ask. Then if they tell you to fill it out on the Internet, do that - if they give you a paper application, fill that out.

Good luck.
