Has he been to a chiropractor?

Well in any case when I had similar pain I got some relief using capsicum patches. I don't remember if they came from a regular drug store or an herbal store. It wasn't great, but it helped ;-)

easy enough to fix, if it is a pinched nerve you need to massage your fingertip over the area til you find a highly intense, painful spot, that is where the pinched nerve is... you then circle your finger into it and press (press it out) and hold for a few moments, rub out the area and try it again. He is likely to have a similar spot on his neck upon the same side.

You will also want to check if his posture has anything to do with it... is he able to turn his head from shoulder to shoulder and as easily from one side to the next, are his shoulders even when he stands, does it look like he holds his body differently from one side in comparison to the next? If so, a good chiropractor or massage therapist will probably be the best way to go, there grip on things are often completely different than a doctor's view.

two suggestions:
1) get a dry cleaning bag, the big kind that is used to cover dresses or suits if possible. lay if flat over a large, slightly damp towell (not at all wet) and pour a lot of CRUSHED ice in it. wrap the towell around the cellophane and ice lice a burrito. Have your boyfriend lay down somewhere comfortable, with the ice "burrito" pack under the hurting shoulder. bring the ends up and over so that his shoulder area is completey surrounded with the ice pack. He will be comfortable, not cold, in a minute or so. Have him rest on this for 20 minutes. remove the ice pack, and give him a gentle massage with oil or lotion, to bring blood back into the area...or use a heating pad. Do this four times a day, or more.
ALTERNATE METHOD
2) take some small dixie cups (like for mouthwash), and freeze water in them. Peel the paper off frozen ice in the tiny paper cup. hold it with a wash cloth. use several towells near your boyfriend to keep the dripping water from melted ice from driving him crazy. Start a circular massage, with pretty good pressure, at the most painful area of his shoulder. Small circles in the same area, not more than a minute or two at the most, then move out and around with your circular ice massage, until the entire area that hurts is basically numb. follow with a lotion or oil massage to bring the healing blood back into the area. Do several times a day, as needed.

The effectiveness of both styles is due to the same thing so pick the one you or your boyfriend would prefer. The ice will heal numb area and reduce inflammation. The increased blood supply to warm area up will speed healing

From your description, this is an inflamed tissue response. There is no nerve to pinch, which would cause such symptoms as you present. There is rather severe spasticity of the muscles of the area. Causing congestion to the area, already inflamed and swollen. The degree of pain is intensified resultant to the muscles under the blade being also in rather severe spasm.

As is often the case, the old adage applies... When all else fails, see a chiropractor. Something, if done originally would have prevented the intense degree of inflamation.

Move to Canada and bask in the glorious National Healthcare. You should really see a doctor if you have a pinched nerve, it's a very serious condition.
