If it gets to the point that you can't breathe, and you don't have a rescue inhaler or it doesn't work, then your brain and heart die from lack of oxygen. It's pretty rare, but it happens.

During very severe attacks, an asthma sufferer can turn blue from lack of oxygen, and can experience chest pain or even loss of consciousness.
Just before loss of consciousness, there is a chance that the patient will feel numbness in the limbs and palms may start to sweat.
Feet may become icy cold.
Severe asthma attacks, which may not be responsive to standard treatments (status asthmaticus), are life-threatening and may lead to respiratory arrest and death.

Um, not being able to breathe, and not having the proper medication on hand, to open your air passages fast enough.

Sometimes inhalers don't work, depending on your condition and you may have to go to a nearby hospital to receive an oxygen or albuterol treatment. My Mother and brother have asthma and have had to deal with this condition for years. It sucks.

Asthma effects you lungs. During a asthma attack you lungs can become so inflamed that the inflammation can constrict you breathing. Then you brain dies, then goes you heart then your dead! Most people however use a inhaler that helps with keeping their lungs open.

If one deteriorates bad enough their airway can close up preventing oxygen from being inhaled.
