Do you want to go to a large gauge immediately when the piercing is done, or do you want to stretch a current piercing?
If you want to go straight to a large gauge with a new piercing, you have 2 options, scalpeling, or dermal punch. With a scalpeled piercing a small section of tissue is removed from the lobe and a flesh tunnel inserted. With a dermal punch piercing a circular chunk of tissue is removed with a dermal punch like used for biopsys and a flesh tunnel is inserted.
If you already have the piercing and want to stretch to a larger gauge to use flesh tunnels, buy a good set of tapers (stainless steel) and slowly move through the gauges. In other words, stretch to one size, wait a month or two, then stretch to the next until you are at the gauge you would like your flesh tunnels to be. Keep in mind though that large gauge earrings should always be looked at as a permanent modification, especially if they are scalpeled or dermal punched.

You start with a small plug, then a few weeks later, after that one has healed you put a slightly bigger one in. Mine are 10mm and staying that way. I started them when I was 13 (I'm 19 now and have hated them for 2 years) and the biggest they were was 23mm, shrunk them down and 10 is the smallest they'll go. Wish I'd never done it.

oooooo i love flesh tunnels. really really want one but ive already got 3 piercings in each ear lobe so there isnt enough space!!!

you can either scalpel them (ie cut them to size - needs a practised professional!) or have a standard piercing and stretch it up

you have your ears pierced and once they are healed you use a taper to slowly make the holes bigger.
