There are at least two ways I know of. One is with a drill and a tap. The other is with a lathe using a thread cutter.

In the first, a hole is drilled to a specific diameter specified by a required tap. Then the tap is screwed into the hole. A tapping fluid is used to make it go smoothly.

On a lathe, the hole is drilled much as above. Then the special thread cutter is cut into the hole using the lathe's ability to advance at a fixed rate. The machinist uses a chart to tell him the speed to use and the thread it cut accordingly.
