I don't quite understand the phrasing of your last sentence. Yes, you may be able to use a doctor's certificate, however, this alone will not be enough and doctors will not backdate illnesses in Australia (if you saw a doctor about a cough on Sunday they would not be able to say you were sick on Saturday or any day before that).

DIAC will also take a close look at what kind of sickness he had, if he saw a specialist, if he is on any medication for his illness, will be able to recover fully enough to qualify for an extension/keep his visa.

Did your friend inform his teachers that he was sick or try to get his friends to hand in his assignments in his absence? That might help his case.

The medical certificate must be genuine to prove that your friend was absent due to sickness.
