Have someone drive you through the freeway twice and talk you through the process. Take the wheel on the 3rd try but still with someone guiding you through.
Consider taking the Smith System Advanced Defensive Driving Course. As safety coordinator in a multi-national corp., I took several courses and found Smith System to be the best.

try short drives when the freeway is less busy. Plan what exits you need to use aand get in the exit lane early . Dont panic if you miss an exit, just have an alternate route . If you are overwhelmed pull off on the breakdown lane , shut off the engine and just relax for a while .

stay off the freeway until you are a more confident motorist. If you must get on the freeway, remeber to breathe and try to relax.
Also get some practice in on the freeway when it isn't so busy, like at night or on Sundays.

Steer smoothly at all times.

lots of people have this my mother 87 does not dtrive anymore but had this same phobia. use the back roads she did for years and got the things she needed. just was not the type of person to make long distance highway trips.
it is okay, do not feel any anxiety.

haha my grandma had that problem to just make sure you have enough time to get where your going WITHOUT taking the freeway. there is always another way!
