If you believe it actually is a name, it may be a shortened name. Yamaguchi might be shortened amongst friends so the person becomes yama-chan or simply yama. Without seeing the kanji its difficult to tell, but it def doesn't look like a complete name be it a 1st or last. (my guess would be a shortened last name).

Never heard of kosa as a first or last name in Japanese.

Actually, to my knowledge people are named by the literal meaning. If Kosa is a clan, that it's a last name (first/family name) I've never heard of that name though.

Accept the advice from experienced persons on this list.
Identify your specific ancestors, and use valid records to find where they came from.
If you really want to know YOUR ancestry, you are sabotaging yourself, by attempts to generalize by "where a name comes from".
