Death is not a consequence of life. Death is PART of life. Assuming that you believe that your soul continues, there's more than just the physical death of your body. Your consciousness continues on in a purely spiritual life.

That depends upon the belief system that you subscribe to.
It could be the absolute end.
It could be rebirth to those who follow a faith that includes reincarnation.
It could judgement, followed by hell, purgatory, or heaven, depending on how you lived your life and what you believed in your heart.
