Concealed carry means not in plain view. Whether or not it is loaded is irrelevant. If it is illegal to carry concealed in your State (or where you happen to be at the moment, like the polling place, a Federal building that restricts possession of weapons or whatever), that it is unloaded is NOT a defense from prosecution.
If you are asking: Can you carry an unloaded gun concealed in backpack legally? Answer: Probably not, at least not without the proper concealed carry permit or license issued by your State. Again, the fact that it is unloaded does not preclude it from being a prohibited weapon, to-wit: A handgun. If the local District Attorney takes the charges you will be arrest for unlawfully carrying a prohibited weapon (whether it is loaded or not). If you legally possess a concealed carry permit in your State then it is okay to conceal it in a backpack. Don't walk away from your backpack, though, with your gun in it.

Hidden from view. Usually on your person but some states consider a hidden gun in a vehicle to be a concealed weapon. Check with your local Sheriffs office as they usually handle the concealed weapon permits and should know the legal definition of concealed.

Concealed is not being in full view. As for the unloaded gun in your backpack it depends on where you're standing. In a bank or government building it will be illegal even if you have a concealed carry permit. Most anywhere else is OK as long as your ammo is located somewhere else.

Depends on where you live. A concealed, unloaded gun in New Mexico is legal. If the gun is loaded you must have a concealed carry permit. Also in NM, a concealed weapon is legal while in your vehicle, loaded or not, without a permit. Once you exit the vehicle the weapon must be in plain sight unless you have a concealed carry permit.

Generally speaking, if it is not in plain view, the way policemen wear theirs, it's concealed, regardless of whether it's loaded.
Of course, if it's unloaded it's NFG anyway.
