Easiest way would be to born again. Commit all sins you can think off you will be born in a Muslim family. I guarantee it.

Your previous name in your educational certificates cannot be replaced with your new (present) name. Names in the educational certificates cannot be changed by any means in India, probably in other countries too.

You should approach the District Magistrate (District Collector or the Deputy Commissioner in some Indian states) and apply for a change of name in the prescribed proforma alongwith proof of citizenship/nativity. It will be got enquired by the police to know whether you have any criminal cases pending. Thereafter an order will be passed allowing you to change your name. You will be asked to get that notification of change of name in the official Government Gazette. Thats all.

In all official correspondence thereafter, you may write your new/present name (after embracing Islam) and enclose a copy of that official gazette alongwith your said educational certificates having previous name.

Further guidance may be sought if needed.

mer of muslim)your sins have been eliminated by Allah the only one God so you have no sin like a new born baby.But your good doings are still with you.What certificates do you mean ?Birth certificate,schools certificates and married certificate ?.If you want to change it you must contact to the officers who made those certificates.
But I think those certificates do not need to be changed because those are your history before you become a muslim.
It is better that you have also a certificate of becoming a muslim and muslimah of your wife with the new names both of you.By showing the certicate of becoming muslim the related person who need your cetificates will know your former name in Hindu .Your former certificates are the evidences of your former events.,before you converted your religion and changed your name.

Write to the institutions concerned With proof if they can change it in he certificate and the records maintained by them.Else you issue a notification through the official gazette of your stat declaring that you son of so and so originally a Hindu has converted to Islam from the particular date and shall henceforth be known as so and so .Take copies of It and add to your certificates .You can a so issue a notification in popular news paper and take copies of it in the certificates.

you must ask the concerned institutes and departments to give you the new certificate, confiscating the old ones, and you can have your new name on the new certificates. I think you might need some paper advertising or some other prrof of ur name change.
