I have an 11 week old Rottie puppy. She constantly goes to the bathroom in her cage, even though I let her out all the time throughout the night. She doesn't go to the bathroom otherwise, and I know dogs don't usually go to the bathroom where they sleep. Is there anything I can do to prevent her from doing this?
She doesn't get any food after 6pm, she gets put to bed when I go to bed, at about 1am, and gets let outside lots before then.

I only feed her organic food.
Also, she's going outside pretty often. She goes to the door anytime she has to go out, and I let her out even when she doesn't.
Also, her crate is just big enough for her to turn around in. I have a larger one that I leave her in during the day, when I have to go out, but she's fine in that.
It's just over night. She's only in there for about 5 hours, and she never whines. I have her pretty close to me.
I may just have to take her to the vet.

How big is the cage? If you are trying to crate train a dog, then the cage needs to ONLY be big enough for her to stand up and turn around. Its true that most dogs do not want to go potty where they sleep.. but if they have enough room, then they will.

11 weeks is still very young. I would suggest, since you have already done the controlled feedings, getting blue pads and putting them in her cage. This will help absorb her potty and make clean up easier for you.
Also, in case you haven't already done this...better nutrition helps greatly! The better the food, the more it "sticks" to their insides instead of going right through them. It is worth the money!
Doodle is absolutely right!! I know it is a pain in the rear, especially with a rapidly growing pup, but if she has room to roam...then she will potty in the cage. Yes, it should only be big enough to stand up, turn around, and sit down.

How big is the cage? The dog should only have enough room to turn around in it.
My puppies at 11 weeks had to go potty about once an hour. Maybe yours needs to go out more.

She's just a puppy, and she can't hold her urine for five hours at a time yet. Try to get up once during that five hours to let her out. In a couple of months she'll be mature enough to hold it all night. You don't need to take her to the vet. Every dog is different and grows and learns at a different pace. Be patient with her.

It will take a little while for your dog to understand she's supposed to go outside. Whenever you let her out and she eliminates in the appropriate setting, make sure you immediately praise her. There probably isn't anything you can do immediately tonight to get her to stop going to the bathroom in her cage, but as long as you keep up the training/praise when she goes outside, it'll stop in a week or two. If the dogs are small, they can only hold their bladder for so long. A small dog can usually only hold its bladder for 4-5 hours, so if you are leaving them in the crate for longer than this, you need to find a different set up. Perhaps a doggy door?

She may have worms. Get the vet to check her. My neighbor's dog peed and pooped in his crate, and when they went to the vet, they found out it was caused by a BAD case of worms.
11 week old pups often have worms.

At 11 weeks old they can't hold their bladder for more than a couple of hours, so you may want to put her crate in your room and if you hear her whining, let her out.
You may also want to have her checked out for digestive or urinary tract problems.

Why have a dog if you are going to cage it? If you can't take the time to properly train it and take it out for the bathroom then find a better home for it and get a cat. They go in a litter box and you don't have to cage them.
