Flannel-lined khakis or jeans from LL Bean under my skipants; flannel boxers; a fleece and wool ski hat, wool (knee length)socks, a wool sweater, a turtleneck; a fleece, nylon, and thinsulate ski jacket; fleece glove liners, insulated nylon mittens, a scarf, and goggles.
It's actually not all that heavy or bulky.
It's also important to have a little room in the toes of your socks so air can circulate. Don't get overheated - vent your jacket w/the zipper if you get overheated; sweatiness will later freeze when you cool off.
I also have a tiny, discreet flask in which I keep a little brandy or single malt scotch (but this isn't recommended for extreme or survival conditions). A small thermos of hot tomato soup or cocoa can help (and the food at the lodges is usually expensive).
You can also get electric socks but they're expensive & don't last for more than 2 or 3 wearings.

Base Layer:
Smartwool liner socks. (Very thin. I let my ski boot insulation do its job.); EMS Techwick or Bergalene shirt. EMS Techwick boxers.
Mid Layer:
Usually a technical wool sweater with a 1/2 zipper. On really cold days my mid-layer is a Windbloc fleece or a Primaloft jacket. EMS or LL Bean fleece pants;
Top-Layer:
Warmer days I just wear the Windbloc fleece. On cold days I wear a jacket from Precision Mountaineering. EMS All Mountain Ski Pants or Obermeyer Insulated Ski Pants when it is below 10F.
Accessories:
Outdoor Research Pro Ice mitten shells with Gore N2S Windstopper liners. My helmet is insulated. I tend to wear transitional tint sunglasses instead of goggles, but I have a few pair of goggles...all crappy. Somewhere I have a fleece face warmer, but I rarely wear it too.
This is my standard clothing line-up. It varies a lot depending on the conditions and how agresssive I'm skiing. Somedays my Mid-Layer is a sweater and I stash a lightweight rain jacket in my pack...just in case.
Ditto on the flask, Andrew. I keep it stocked w/ Old #7 myself.
